Job Description – RN Care Manager / Field Case Manager

Location: Hybrid – Remote & Field-Based in Northeast Ohio (Loraine, Medina, Lake, Cuyahoga areas)

Duration: 6 Months + Possibility for extension or conversion

PR:$45-50.00/hr

Training Schedule:

  • Monday–Friday 8:00 AM – 5:00 PM EST
  • 4 weeks of virtual training
  • Cameras must remain on
  • No time off permitted during training

Position Overview

  • Develops, assesses, and facilitates complex care management activities for members with primarily physical health needs to promote high-quality, cost-effective healthcare outcomes.
  • Responsible for creating personalized care plans, coordinating services, conducting member visits, and educating members and families to support independence and overall wellness.

Key Responsibilities

  • Review emails, voicemails, tasks, and urgent member needs daily
  • Coordinate and facilitate complex care management activities
  • Conduct 5–7 member visits weekly
  • Develop individualized care plans and coordinate services
  • Complete required documentation within 24 hours of visits
  • Collaborate with interdisciplinary teams and healthcare providers
  • Educate members and families regarding healthcare plans and services
  • Work independently to manage field visits and scheduling
  • Ensure compliance with turnaround times, training requirements, and company policies

Education

  • Degree from an Accredited School of Nursing OR Bachelor’s Degree in Nursing

Experience:

  • 2–4 years of experience in Nursing, Case Management, Home Health, Behavioral Health,

Licensure

  • Active Ohio RN License (Compact license not accepted)
