About the Role

We are aphysician-owned, Medicare-certified hospice agency serving patients acrossStark, Summit, Tuscarawas, and Portage counties. Our mission is to providedignified, compassionate, and personalized end-of-life caresupporting both patients and their families through one of lifes most meaningful journeys.

As anRN Case Manager, you will play a vital role in guiding each patients care plan, ensuring comfort, dignity, and quality of life. Youll work closely with our interdisciplinary teamincluding physicians, social workers, chaplains, aides, and volunteersto deliver holistic hospice care in the setting the patient calls home.


Key Responsibilities

  • Manage an assigned caseload of hospice patients in home and facility settings across Stark, Summit, Tuscarawas, and Portage counties.
  • Conduct comprehensive initial and ongoing assessments, identify patient/family goals, and develop individualized plans of care.
  • Provide symptom and pain management through nursing interventions and coordination with the hospice medical director.
  • Collaborate closely with the interdisciplinary group (IDG) to ensure seamless, holistic care and regulatory compliance.
  • Educate patients and families on disease progression, comfort measures, medications, and emotional support resources.
  • Participate in hospice admissions, recertifications, and discharge planning.
  • Maintain accurate and timely documentation in the electronic health record (EHR) system per Medicare and CHAP standards.
  • Participate in on-call rotation and respond promptly to patient/family needs after hours as assigned.
  • Serve as a compassionate advocate for the hospice philosophy of dignity, comfort, and family support.


Qualifications

  • Active Registered Nurse (RN) Licensein the State of Ohio (required)
  • Minimum 1 yearof hospice, palliative care, or home health experience preferred
  • Strong clinical assessment, organizational, and communication skills
  • Valid drivers license, reliable vehicle, and willingness to travel within Stark, Summit, Tuscarawas, and Portage counties
  • Ability to work independently while collaborating within a compassionate, supportive team environment
  • Current CPR/BLS certification
