Key Responsibilities

• Provide individual and family psychotherapy tailored to patients facing illness-related stress, grief, and mental health conditions such as anxiety, depression, and PTSD.
• Conduct comprehensive assessments of mental health status, including emotional, behavioral, and social functioning, to inform treatment strategies.
• Develop personalized treatment plans that are evidence-based and culturally sensitive, ensuring conformity with best practices in mental health care.
• Regularly monitor patient progress and adapt treatment plans based on patient feedback and the insights of other healthcare professionals.
• Collaborate effectively with physicians, psychiatrists, nurses, and other members of the healthcare team to deliver a coordinated approach to patient care.
• Educate patients and families on mental health issues, treatment pathways, and available community resources for additional support.
• Uphold all ethical standards of social work practice, ensuring patient confidentiality and the informed consent process is meticulously followed.

Patient Population / Client Focus

This position focuses primarily on providing care to patients in hospice who are grappling with not only chronic pain and serious illness but also potential mental health challenges. Our social worker will play a critical role in addressing the emotional turmoil associated with end-of-life scenarios, and in supporting families through the grief process.

Qualifications

• A Master’s degree in Social Work from an accredited educational institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Minimum of 2-3 years' experience in providing mental health services, specifically within a clinical or hospice care environment.
• Strong clinical assessment skills and a proven ability to formulate evidence-based treatment plans effectively.
• Exceptional communication skills to facilitate meaningful relationships with patients and collaborate with healthcare teams.
• Proficient in using electronic health record (EHR) systems and telehealth platforms for maintaining accurate patient records and coordinating care.
• Deep understanding of mental health conditions and a compassionate approach towards patients and families dealing with them.

Preferred Experience

• Experience operating in a hospice or palliative care setting is highly desirable.
• Familiarity with community resources and support networks available for patients and families in need.
• Background in conducting patient education programs related to mental health treatments is a plus.

Compensation and Benefits
