Key Responsibilities

• Conduct comprehensive assessments of psychosocial needs focusing on emotional, social, and mental health factors.
• Develop and implement personalized care plans in collaboration with clients and their families, tailoring interventions to meet specific goals.
• Provide direct counseling and emotional support to seniors, assisting them through various life transitions such as bereavement, relocation, or managing chronic illnesses.
• Advocate for clients to access necessary community resources, ensuring they receive quality services and support.
• Collaborate with interdisciplinary teams, including healthcare professionals, nurses, and caregivers, to ensure holistic care delivery.
• Regularly monitor client well-being through virtual visits or phone check-ins, addressing any emerging concerns proactively.
• Facilitate conflict resolution and effective communication among family members involved in the care process.
• Educate families about available community resources, support groups, and other programs to foster social engagement.
• Accurately document client assessments, care plans, and progress notes while staying updated on best practices and regulations in social work.

Patient Population / Client Focus

The primary clientele consists of seniors and their families who may experience complex emotional and mental health issues. Our focus includes empowering clients to regain control over their lives, with particular attention to managing anxiety, depression, and adjusting to life changes. Qualifications

• A Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of professional experience in social work, particularly within a community-based or geriatric settings.
• Strong understanding of social work principles and aging-related issues.
• Excellent communication and interpersonal skills to engage clients and families effectively.

Preferred Experience

• Previous experience in telehealth service delivery or remote counseling.
• Familiarity with community resources available for seniors, including mental health programs and support services.
• Experience working as part of an interdisciplinary team.

Compensation and Benefits

The salary range for this role is between USD 70,000 and USD 100,000 per year, commensurate with experience and qualifications.
