  • Advocate for clients to access necessary community resources, ensuring they receive quality services and support.
  • Collaborate with interdisciplinary teams, including healthcare professionals, nurses, and caregivers, to ensure holistic care delivery.
  • Regularly monitor client well-being through virtual visits or phone check-ins, addressing any emerging concerns proactively.
  • Facilitate conflict resolution and effective communication among family members involved in the care process.
  • Educate families about available community resources, support groups, and other programs to foster social engagement.

Compensation and Benefits
