Position Overview

Work Environment

This full-time position offers a flexible remote work structure, with occasional on-site responsibilities at the Community Center or local schools as needed. Typical working hours will be Monday to Friday, from 8:00 AM to 4:30 PM, though you may be required to adjust your hours for client visits or emergencies. You will work alongside a multidisciplinary team of healthcare professionals, including nurses and caregivers, to provide well-rounded support to our clients.

Key Responsibilities

As the Licensed Clinical Social Worker, you will take on a variety of responsibilities, including:

• Conducting thorough assessments of seniors' psychosocial needs, examining emotional, social, and mental health factors.
• Developing personalized care plans in collaboration with clients and their families, tailored to their specific goals and circumstances.
• Providing one-on-one counseling and emotional support, assisting seniors and their families through life transitions such as loss, illness, or relocation.
• Advocating for clients’ rights and ensuring they have access to necessary community resources and services.
• Working closely with interdisciplinary teams to coordinate care and ensure continuity of services.
• Performing regular wellness checks and safety assessments in seniors' living environments.
• Facilitating communication among family members to resolve conflicts regarding care and management.
• Educating clients and families about available community programs, support groups, and resources to enrich their social support systems.
• Maintaining comprehensive case documentation, including assessments, care plans, and updates on client progress in electronic health record (EHR) systems.

Qualifications

The ideal candidate for this position will possess the following qualifications:

• A Bachelor’s Degree in Social Work (BSW) from an accredited institution; a Master’s Degree in Social Work (MSW) is preferred.
• Current and valid state license as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of relevant social work experience, ideally in a geriatric or community center setting.
• Profound understanding of social work principles, particularly those concerning aging populations and community resources.
• Exceptional communication and interpersonal skills necessary for effective collaboration within multidisciplinary teams.
• Strong problem-solving skills and the ability to empathize with clients’ circumstances and feelings.
• Familiarity with electronic health record (EHR) systems.

Preferred Experience

While not mandatory, candidates with experience in geriatric social work, case management, or providing group therapy to seniors will be given preference. Knowledge of local community services and support systems will also be beneficial.

Compensation and Benefits

This position offers a competitive salary range of USD 60,000 to 100,000 annually, commensurate with experience and qualifications. In addition to monetary compensation, our comprehensive benefits package includes:
