WHAT ARE WE OFFERING? 

We are looking for a skilled and compassionate Clinical Nurse to join our dedicated team within the Rural Support Service. The Clinical Nurse role will deliver high-quality, person-centred nursing care and case management to clients living with chronic and complex conditions through an innovative virtual care model. As part of the Remote Health Monitoring Service, you will use your clinical expertise to assess, monitor and support clients, developing tailored care plans that promote self-management, improve health outcomes and enable people to confidently manage their health needs. 

This rewarding role provides the opportunity to work collaboratively with consumers, carers, multidisciplinary teams and health services to deliver coordinated, evidence-based care. You will contribute to the continued development of digital health services, support clinical education and quality improvement initiatives, and provide professional leadership to enhance the delivery of accessible, connected and responsive healthcare. 

WHO ARE WE? 

The RSS is a business unit in SA Health that delivers a range of enabling services, such as workforce planning and recruitment, digital health, finance, infrastructure, procurement, and clinical governance support. Our purpose is to strengthen and sustain healthcare delivery in rural and regional areas by providing consistent, efficient, and specialist support functions to the six regional local health networks (LHNs) across South Australia.  We employ around 260 people who work in 37 regional, rural and remote SA locations, or in the RSS's three Adelaide offices.

  • Provide comprehensive nursing assessment, care coordination and case management for clients living with chronic and complex conditions through a virtual care model. 
  • Monitor client health information, identify changes in condition and adjust care plans to support safe, effective and ongoing care.  

Deliver clinical advice, health education and self-management support to empower clients to achieve their health goals.  

Collaborate with multidisciplinary teams, consumers and external services to coordinate care and improve health outcomes.  

Contribute to digital health service development, clinical education, quality improvement activities and evidence-based practice.
