Key Responsibilities

Your role will involve a variety of responsibilities aimed at improving the mental well-being of patients. Specific duties include:

• Conducting thorough clinical assessments to evaluate patient needs, which will inform individualized treatment plans.
• Delivering psychotherapy through evidence-based modalities such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT) tailored to the unique circumstances of patients and families in hospice care.
• Maintaining comprehensive clinical documentation that meets HIPAA standards, ensuring both compliance and quality of care.
• Collaboration with a multidisciplinary treatment team, including physicians, psychologists, and clinical staff, to provide cohesive care and a multidisciplinary approach to patient treatment.
• Engaging in regular case conferences to discuss patient progress, treatment efficacy, and plan modifications.
• Providing education to patients and families regarding mental health considerations, coping strategies, and available community resources during end-of-life care.
• Staying informed of emerging research and clinical practices in mental health to ensure evidence-based approaches are integrated into therapy.

Patient Population / Client Focus

You will be primarily working with patients experiencing a range of complex emotional and psychological issues related to end-of-life circumstances. The focus will be on individuals and families needing support with grief, bereavement, caregiver stress, and overall mental health challenges associated with terminal illness. This unique patient population requires sensitivity, empathy, and specialized skills to effectively address their needs during this critical period.

Qualifications

The successful candidate will meet the following qualifications:

• A Master's degree in Social Work from an accredited program.
• A valid and active license to practice as a Licensed Clinical Social Worker (LCSW) in the state of Georgia.
• A minimum of three years of experience in a clinical setting, specifically providing therapeutic services related to mental health.
• Demonstrated knowledge of solution-oriented psychotherapy interventions and best practices.
• Exceptional communication and interpersonal skills, capable of fostering strong therapeutic alliances with patients.
