Job Summary
September through December
Lead a team of Care Navigators that are charged with processing referrals to secondary care, outside of Crossover Health (specialists, imaging, community resources, employer benefits).
Job Responsibilities
  • Provide training, support, and guidance to the Care Navigation team.
  • Maintain high quality specialist, community service, and employer benefit directories.
  • Audit data integrity, including patient feedback, and proper documentation of referrals.
  • Be a point of contact for the CN team for troubleshooting or escalating issues.
  • Manage the staffing schedule for efficiencies and optimal staffing coverage.
  • Establish and maintain relationships with identified service providers.
  • Assist with training relevant stakeholders on the external referral process.
  • Responsible for day to day staffing and appropriate coverage.
  • Assist the team in processing a determined amount of referrals.
  • Serve as a role model for the Care Navigation team to achieve our quadruple aim: decreased healthcare spend, increased quality, excellent patient and provider experience.
  • Performs other duties as assigned

Required Qualifications
  • Minimum 3 years experience working in the healthcare field
  • Minimum 1 management or supervisory experience
  • Bachelor's degree or relative medical experience
  • Experience in office management or administrative role
  • Excellent communication and people skills
  • Ability to multitask and prioritize
  • Self-starter with strong problem-solving skills
  • Excellent organizational and time management skills

Physical Job Requirements
  • May require standing, walking and sitting for extended amounts of time.
  • Manual and finger dexterity and hand-eye coordination
  • Requires corrected vision, hearing and speech within normal ranges. Must be able to effectively communicate with patients and team members.

The base pay range for this position is $56,294.00 to $73,182 per year. Pay range may vary depending on work location, applicable knowledge, skills, and experience. This position will be eligible for a comprehensive benefits package that includes Medical, Dental, Vision and 401K.
