• Conduct thorough assessments and develop individualized treatment plans that address the diverse needs of clients, including pre-crisis and crisis management strategies.
• Maintain timely and accurate documentation in compliance with regulatory standards and organizational best practices.
• Utilize evidence-based screening tools as appropriate, collaborating with clinical teams to ensure comprehensive behavioral support plans are implemented effectively.
• Participate actively in licensing and accreditation processes to uphold the highest standards of care.
• Serve as a vital liaison with relevant service systems, including state agencies and community partners, to coordinate support services for clients.
• Facilitate open communication within the team and with external stakeholders, aligning with organizational goals and community needs.
• Establish and nurture relationships with local, state, federal, and private organizations to enhance program visibility and secure funding sources.
• Engage in continued professional development, attending conferences and training to stay updated on best practices in mental health care.
• Promote the mission, values, and philosophy of Community Outreach Mental Care in all aspects of your work.
• Perform additional tasks as assigned by the Clinic Director to support clinic operations and client care.

The LMHC will work with a diverse client base, including individuals facing emotional distress, substance use challenges, developmental disabilities, and those recovering from brain injuries. Your role will be crucial in implementing supportive interventions and creating a safe environment where clients feel heard and valued. By delivering tailored care, you will help clients build resilience and progress toward their personal wellness goals.

• A Master’s Degree in a behavioral health field is required, ensuring a strong foundational knowledge of mental health practices.
• An independent LMHC license issued by the state board is mandatory for this role.
• Proficient in maintaining clinical records and preparing detailed reports that reflect client progress and treatment effectiveness.
• Strong collaborative skills, with the ability to work effectively within an interdisciplinary team setting.
• Effective communication skills, both verbal and written, enabling clear expression of ideas and compassion for clients.
• Sound judgment and the ability to make independent decisions based on clinical knowledge and best practices.

• Previous experience in a community mental health setting or clinic is highly desirable, providing insight into integrated care models.
• Familiarity with evidence-based therapeutic practices and client-centered treatment approaches is a plus.
• Experience working with the specific populations served by Community Outreach Mental Care will be considered an asset.
