As a Licensed Clinical Social Worker at our remote clinic, you will take on several critical responsibilities that include:

• Conducting comprehensive clinical assessments to understand patients' mental health needs and developing individualized treatment plans accordingly.
• Employing evidence-based therapeutic modalities such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) to provide impactful interventions tailored to each client.
• Maintaining meticulous and timely clinical records that adhere to HIPAA regulations and organizational standards, ensuring confidentiality and compliance throughout.
• Collaborating closely with an interdisciplinary treatment team, including psychiatrists and psychologists, to deliver coordinated and effective care to patients.
• Engaging in regular case consultations and peer-review sessions, sharing insights that contribute to enhancing the overall quality of care.
• Providing valuable education and support to patients and their families on mental health issues and available community resources, thereby helping them navigate their treatment options.
• Staying informed on the latest developments in mental health research and clinical practices, integrating new insights into your treatment strategies.

You will primarily work with individuals facing a range of mental health challenges within the hospice care context. This role demands a strong emphasis on understanding the complex emotional and psychological needs of patients undergoing end-of-life transitions, as well as providing supportive care for their families during these critical times. Your interventions aim to alleviate suffering, enhance quality of life, and promote a sense of dignity and peace for both patients and their loved ones.

• A Master’s degree in Social Work from an accredited institution, demonstrating a solid academic foundation.
• A valid and active state licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Specialist Clinical Social Worker (LSCSW), confirming your professional qualification.
• At least 2 years of clinical experience in providing psychotherapy services, particularly in a hospice or end-of-life care setting.
• Proficiency in solution-focused therapy techniques that prioritize practical interventions and client involvement.
• Commitment to exceptional communication and interpersonal skills that facilitate trust and rapport with patients.
• Strong analytical and problem-solving capabilities that enable clinical judgment and effective treatment planning.

• Experience working in hospice, palliative care, or similar healthcare settings that provide mental health support to terminal patients.
• Familiarity with electronic health record (EHR) systems to streamline documentation and patient tracking.
