• Conduct comprehensive mental health assessments, carefully considering each patient's existing medical conditions to address their mental well-being fully.
• Design and implement tailored integrated treatment plans that account for both psychiatric and medical aspects of care.
• Facilitate both individual and group therapy sessions, applying evidence-based techniques and approaches to foster patient growth and resilience.
• Work collaboratively with medical providers to ensure seamless coordinated care, focusing on comprehensive treatment strategies to optimize health outcomes.
• Provide immediate crisis intervention and support for patients experiencing mental health emergencies, utilizing best practices and clinical judgment.
• Educate and empower patients on the interplay between mental health and physical well-being, fostering self-management and understanding of their medical conditions.
• Maintain meticulous and confidential documentation of patient interactions while adhering to strict legal and ethical standards.
• Engage proactively in team meetings, case conferences, and interdisciplinary care planning discussions to support a collaborative approach to patient care.
• Stay informed about the latest research, best practices, and advancements in integrated mental health care to enhance the clinical services provided.

The therapist will primarily serve individuals facing a multitude of mental health challenges, particularly those with co-occurring medical conditions such as autoimmune disorders, chronic pain, and substance use disorders. This patient demographic often requires an empathetic and skilled approach to treatment, understanding the complex interaction between their physical and mental health needs.

• A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Current and valid licensure as an LMHC, LCSW, LCSW-R, Licensed Psychoanalyst, Psychologist, or LMFT in the state of New York.
• Demonstrated experience in addressing the mental health requirements of individuals with co-occurring medical conditions, showcasing a comprehensive understanding of integrated care.
• Exceptional interpersonal and communication skills to effectively connect with patients and collaborate with a diverse team of healthcare professionals.
• The ability to maintain professional boundaries and adhere to ethical guidelines while ensuring patient confidentiality at all times.

• Experience utilizing electronic health record systems for documentation and patient management.
• A proven track record of working in interdisciplinary teams to enhance patient care.
• Familiarity with remote therapy tools and best practices for virtual engagement.
