• Conduct thorough mental health evaluations, including psychosocial assessments and risk assessments, to develop a comprehensive understanding of each patient's needs.
• Engage collaboratively with patients to create individualized treatment plans that encompass their goals, psychiatric history, and cultural values.
• Deliver therapy in various formats such as individual, couple, family, and group settings, utilizing evidence-based approaches like CBT and DBT to facilitate healing and coping strategies.
• Provide crisis intervention services when required, ensuring immediate support is given to patients in acute distress, and coordinate with other healthcare professionals for seamless patient care.
• Maintain accurate and current clinical documentation, including progress notes and treatment plans, in compliance with organizational policies and legal standards.
• Offer psychoeducation to patients and their families, empowering them with knowledge to manage their mental health challenges effectively.
• Work closely with a multidisciplinary team to ensure comprehensive care planning, addressing the holistic needs of patients.
• Participate in routine evaluations of treatment outcomes to assess therapy effectiveness and adjust plans as needed.
• Stay informed about the latest developments in mental health research and best practices to ensure high-quality clinical service delivery.
• Adhere to ethical guidelines and maintain the confidentiality of patient information rigorously.

You will primarily work with individuals facing end-of-life issues, providing vital support that respects their unique circumstances. Our patients come from diverse backgrounds and present varying psychological challenges, ranging from grief and loss to anxiety and depression related to terminal conditions. Your expertise will contribute significantly to improving patient outcomes and quality of life during this challenging phase.

• A Master's degree in Social Work is required for this position.
• An active New York state license as a Licensed Clinical Social Worker (LCSW).
• Demonstrated ability to work independently, as well as cooperatively within a multidisciplinary team.
• Strong interpersonal and communication skills, with the capacity to build rapport with diverse patient populations.

• Prior experience in hospice or palliative care settings is highly desirable.
• Experience with various therapeutic modalities, particularly CBT, DBT, and SFBT.
• Familiarity with documentation standards and electronic health record (EHR) systems is a plus.
