Work Environment

This role operates in a flexible remote setting, allowing you to deliver high-quality counseling services from the comfort of your home while focusing on an outpatient population. Key Responsibilities
  • Conduct initial and ongoing assessments with clients to collaboratively identify their personal goals and available support systems.
  • Develop individualized rehabilitation plans that reflect client preferences, educational backgrounds, aptitudes, and aspirations for future employment.
  • Actively collaborate with a diverse team of healthcare providers, including physicians, occupational therapists, and psychologists, to design and implement effective rehabilitation strategies that address specific client needs.
  • Maintain detailed case files and documentation for each client, including personal information, eligibility criteria, insurance details, therapeutic services rendered, and ongoing contact narratives.
  • Regularly monitor and document client progress via progress notes and treatment updates to adjust care plans as necessary, ensuring that goals are met effectively.
  • Provide continuous support to clients attending job training and occupational placement programs, addressing any challenges that arise during their engagement.
  • Involve clients' families in discussions to identify additional support mechanisms for facilitating clients' journeys toward recovery.
  • Conduct timely needs assessments and generate reports to document the evolving needs and achievements of your clients.
  • Deliver informed recommendations and referrals to external resources, ensuring a well-rounded support system for each client.
Patient Population / Client Focus

Qualifications
  • Master’s Degree in counseling, social work, psychology, marriage and family therapy, or a related field.
  • Current and valid Licensed Mental Health Counselor (LMHC) licensure.
