• Conduct thorough mental health assessments, evaluating patients' emotional, behavioral, and social functioning.
• Design and implement individualized treatment plans that are evidence-based and culturally sensitive, addressing disorders such as anxiety, depression, and PTSD.
• Provide psychotherapy and other mental health services, utilizing therapeutic modalities that align with best practices.
• Collaborate with healthcare professionals, including psychiatrists, primary care physicians, and nurses, to create a holistic approach for patient care.
• Monitor patient progress through regular follow-ups, adjusting treatment plans as needed to maximize effectiveness.
• Educate patients and their families about mental health conditions, treatment options, and available community resources to facilitate informed decision-making.
• Maintain ethical standards of practice, ensuring confidentiality and informed consent throughout the therapeutic process.

The LCSW will primarily work with patients in our Hospice Center, supporting individuals facing terminal illnesses and complex psychological challenges related to end-of-life issues. This position requires sensitivity, empathy, and the ability to provide support during an incredibly vulnerable period for both patients and their families. The LCSW will strive to enhance quality of life through therapeutic interventions and compassionate care.

• A Master's degree in Social Work from an accredited institution.
• Current state licensure as a Licensed Clinical Social Worker (LCSW).
• At least 2 years of experience in providing psychotherapy and mental health services, ideally in a hospice or palliative care setting.
• Superior communication and interpersonal skills, enabling effective relationship-building with patients and collaboration with healthcare teams.
• Proficient in evidence-based treatment methodologies and the ability to tailor interventions to fit each patient’s unique context.
• Experience utilizing electronic health records (EHR) systems and various healthcare software for efficient record-keeping and coordination.
• Deep knowledge of mental health disorders and the specific challenges encountered by patients in palliative care.
• Commitment to confidentiality and adherence to professional ethical standards.

• Experience in hospice care, palliative care, or working with patients facing chronic illness.
• Familiarity with community resources and support networks for mental health and end-of-life care.
• Experience in providing family counseling sessions, enhancing familial support systems.
• Prior training in grief counseling or trauma-informed approaches.
