• Conduct comprehensive biopsychosocial assessments to understand each patient’s and family's needs, including their background, current circumstances, and treatment goals.
• Provide individual and family therapeutic interventions, utilizing evidence-based modalities such as cognitive-behavioral therapy (CBT) and mindfulness-based approaches, specifically adapted for end-of-life care.
• Develop and implement personalized therapeutic treatment plans aimed at addressing complex emotional and psychological issues faced by hospice patients.
• Collaborate with interdisciplinary teams, including physicians, nurses, and chaplains, to ensure comprehensive care that aligns with each patient's hospice plan.
• Engage in case management duties, which include monitoring patient progress and adapting interventions based on evolving patient and family needs.
• Foster therapeutic relationships characterized by trust and a collaborative spirit, ensuring patients and families feel supported throughout their journey.

In this role, you will work primarily with hospice patients and their families, addressing a wide range of emotional and psychological challenges associated with terminal illnesses. This may include helping patients navigate existential concerns, grief, and loss, as well as assisting families in coping with anticipatory grief and complicated dynamics that may arise during this difficult time.

• A Master’s degree in Social Work (MSW) from an accredited program.
• Current active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Oregon.
• Strong clinical skills, including proficiency in assessment, treatment planning, and intervention delivery.
• Exceptional communication skills with a demonstrated ability to empathize and connect with diverse client populations.

• At least two years of clinical social work experience, particularly in hospice, palliative care, or a related field.
• Experience with trauma-informed care and advanced therapeutic techniques related to grief and loss.
