The successful candidate will be responsible for:

• Conducting comprehensive clinical assessments to understand patients' needs and formulating effective treatment plans based on solution-focused methodologies.
• Delivering evidence-based therapeutic interventions, ensuring they are personalized and responsive to the evolving needs of patients.
• Documenting clinical encounters meticulously in compliance with regulatory and organizational standards.
• Collaborating with an interdisciplinary team, including physicians, nurses, and other healthcare providers, to deliver holistic care.
• Participating actively in case consultations and peer review meetings to foster collaboration and improve treatment outcomes.
• Educating patients and their families about mental health, coping strategies, and resources available to them.
• Continuously updating knowledge on the latest research and best practices in mental health treatment.

This position emphasizes working with patients in a hospice setting, many of whom face end-of-life challenges. You will support individuals through grief, anxiety, depression, and other emotional responses related to their circumstances. Your role will be critical in enhancing their quality of life and providing a safe space to discuss their fears, concerns, and hopes during this sensitive time.

To qualify for this role, applicants must possess:

• A Master’s degree in Social Work from an accredited institution.
• Valid LCSW license in the state of Illinois.
• A minimum of three years of clinical experience, with a focus on psychotherapy in inpatient or outpatient settings.
• Proficiency in solution-based therapeutic interventions and a commitment to evidence-based practices.
• Strong interpersonal communication skills and the ability to build rapport with patients and families.
• A collaborative mindset, capable of thriving in a team-oriented environment.

While not mandatory, preference will be given to applicants with:

• Experience working with hospice or palliative care patients.
• Familiarity with electronic health record (EHR) systems.
• Background in crisis intervention and grief counseling.
