• Conduct thorough assessments to evaluate patients' psychological and emotional needs, and create individualized treatment plans that best address their unique situations.
• Deliver solution-focused therapies and interventions that aim to improve clients' mental well-being and facilitate coping mechanisms.
• Maintain meticulous and confidential records of patient interactions, treatment progress, and outcomes in compliance with HIPAA regulations.
• Engage in active collaboration with multidisciplinary team members, including physicians, nurses, and other healthcare providers, to ensure comprehensive and holistic patient care.
• Stay abreast of current research and best practices in social work and hospice care to enhance treatment efficacy and optimize patient outcomes.

In this role, you will work primarily with patients in a hospice setting who are facing terminal illness. Your clinical focus will be on addressing mental health concerns such as depression, anxiety, and grief, allowing you to assist families in navigating the complexities of end-of-life issues. Your expertise will ensure that clients receive compassionate care tailored to their emotional needs, enhancing their quality of life during this critical time.

• A Master's degree in Social Work from an accredited institution.
• An active LCSW (Licensed Clinical Social Worker) license in the state of Connecticut is required.
• Demonstrated experience in individual and group therapy, with an emphasis on trauma, anxiety, and depression.
• Exceptional communication skills, both written and verbal, coupled with strong problem-solving abilities.

• Prior experience in hospice care or palliative care settings is highly desirable.

We offer a competitive salary ranging from USD 80,000 to 100,000 annually, commensurate with experience, along with performance-based bonuses.
