• Conduct thorough assessments to identify clients' social, emotional, and psychological needs in the context of hospice care.
• Offer individual and group counseling sessions utilizing evidence-based therapeutic techniques tailored for patients facing terminal illnesses.
• Develop, implement, and monitor personalized service plans that align with clients' goals, focusing on comfort and quality of life.
• Advocate for clients’ rights and facilitate access to necessary resources, including community services and support systems.
• Contribute to program evaluation, development, and grant reporting to ensure the efficacy of our services and align with our mission.
• Collaborate effectively with interdisciplinary teams, including healthcare providers, to enhance the responsiveness and effectiveness of client care.
• Participate actively in training sessions, outreach initiatives, and community engagement activities to foster awareness and educate stakeholders about hospice services.

This role primarily focuses on individuals and families receiving hospice care, which encompasses a variety of emotional and physical challenges associated with end-of-life issues. You will be instrumental in providing support to not only patients but also their loved ones, addressing a range of grief, loss, and psychological needs through a trauma-informed care approach.

• A Master's degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Proficiency in utilizing therapeutic methodologies suited for hospice care and end-of-life counseling.
• Demonstrated understanding of social justice principles, advocacy, and the impact of trauma on patient outcomes.
• Exceptional communication, empathy, and organizational skills to manage client relationships and documentation effectively.

• Previous experience working in a nonprofit organization, especially within a healthcare or hospice environment.
• Familiarity with community resources and programs that support individuals facing terminal illnesses.
• Experience in a collaborative, interdisciplinary setting that promotes holistic patient care.
