Key Responsibilities

• Conduct thorough psychosocial evaluations to assess the emotional and psychological needs of patients and their families in hospice care.
• Develop individualized treatment plans tailored to the unique needs and preferences of each patient, focusing on enhancing their quality of life.
• Provide individual and family therapy sessions, guiding patients and families through their emotional journeys with compassion and understanding.
• Utilize evidence-based therapeutic modalities, including cognitive-behavioral therapy and solution-focused therapy, to effectively support patients and their loved ones.
• Collaborate with interdisciplinary teams, including nurses, physicians, and other healthcare providers, to ensure comprehensive patient-centered care.
• Facilitate grief support and psychoeducation groups for families facing loss, helping them navigate their emotions and develop coping strategies.
• Handle crisis intervention and risk assessment, maintaining patient safety and well-being during these challenging times.
• Ensure all clinical documentation meets ethical and legal standards, contributing to both internal records and billing processes.

Patient Population / Client Focus

This role focuses on patients within the hospice setting, typically including individuals with terminal illnesses such as cancer, heart disease, and neurodegenerative disorders. You will work closely with patients and families from diverse backgrounds and ages, dynamically adapting your therapeutic approach to meet their specific emotional needs. Your role is crucial in providing not only psychological support but also facilitating familial bonding and closure as they experience the challenges of end-of-life care.

Qualifications

• A Master's degree in Social Work from an accredited program.
• A valid state license as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of clinical experience in a mental health or hospice care setting.
• Strong understanding and application of evidence-based therapeutic approaches and modalities.
• Exceptional communication and interpersonal skills, with the ability to build rapport quickly with clients.

Preferred Experience

• Experience working in hospice or palliative care settings is highly desirable!
• Familiarity with telehealth technology and remote care methodologies.
• Training in grief counseling or trauma-informed care can be advantageous.

Compensation and Benefits
