What You Will Do


Depending on your discipline and scope of practice, you will:


* Evaluate and/or treat patients in their homes

* Provide individualized rehabilitation interventions

* Develop and follow appropriate treatment plans

* Monitor patient progress

* Educate patients and caregivers

* Provide home exercise and safety recommendations

* Complete required clinical documentation

* Communicate with physicians, caregivers, referral sources, and the OnCare Rehab team

* Deliver professional, compassionate, patient-centered care

* Orthopedic conditions

* Post-joint replacement rehabilitation

* Hip, knee and shoulder conditions

* Neurological conditions

* Stroke

* Current New Jersey PT license

* Graduate of an accredited PT program

* Current New Jersey COTA credentials/license as applicable

* Graduate of an accredited OTA program


CPR/BLS certification is preferred.


Home-health experience is preferred but not required.
