Job description

Job Title: LVN Runner
Reports To: Director of Clinical Services
Schedule: Monday–Friday, 9:00 AM – 5:00 PM
Pay: $100 per day, $60 per visit plus mileage reimbursement
License Required: Active, unrestricted LVN license in the state of California
Position Summary
The LVN Runner provides real-time clinical phone coverage and performs PRN (as-needed) patient visits during business hours. This role bridges gaps in the visit schedule — covering call-offs, urgent patient needs, and same-day requests — while triaging incoming clinical calls and routing them appropriately under RN and Medical Director oversight.
Key Responsibilities
Answer and triage incoming clinical calls from patients, families, and caregivers during business hours
  • Assess symptoms/concerns by phone and determine urgency; escalate to the on-call RN, Case Manager, or Medical Director as needed
  • Perform PRN visits, including but not limited to: symptom management, wound care, medication administration, vital signs, and general nursing assessment
  • Cover unplanned visit gaps (RN/LVN call-offs, same-day admission needs, urgent family requests)
  • Document all calls and visits accurately and timely in the EMR per agency policy
  • Communicate visit findings and care plan updates to the primary Case Manager/RN
  • Coordinate with scheduling/intake staff on same-day visit assignments
  • Maintain compliance with hospice regulations, infection control standards, and agency policies
Qualifications
Active LVN license in good standing
  • Minimum 1–2 years clinical experience; hospice or home health experience strongly preferred
  • Comfortable with phone-based triage and independent field visits
  • Reliable transportation; valid driver's license and auto insurance
  • Strong communication and prioritization skills — this role requires shifting between phone coverage and field visits throughout the day
Physical Requirements
Ability to travel to patient homes/facilities within the service area
  • Ability to lift/carry supplies (up to ~25 lbs) and perform hands-on nursing care

This is a remote position.
