As Sylvan continues to expand its medical weight management services, we are seeking an experienced and collaborative Nurse Practitioner to help build and grow our GLP-1 program. 

Position Summary 

Sylvan Health is seeking a compassionate, motivated, and clinically experienced Nurse Practitioner (NP) to join our growing clinical team as a part-time W-2 employee. This role is ideal for an NP who is passionate about obesity medicine, enjoys collaborating with referring Primary Care Providers and Registered Dietitians, and is excited by the opportunity to help build an innovative, evidence-based medical weight management program. 

This is primarily a patient care role focused on delivering virtual medical weight management visits for patients using GLP-1 medications. The Nurse Practitioner will partner closely with Sylvan Registered Dietitians to provide comprehensive, coordinated care that supports both medication management and sustainable lifestyle change. 

Because this program is in its early stages, the ideal candidate is adaptable, collaborative, and energized by helping build and shape clinical workflows, protocols, and best practices. 

Responsibilities 

  • Conduct comprehensive 40-minute initial patient evaluations for medical weight management 
  • Provide 20-minute follow-up visits focused on medication management, titration, monitoring, and ongoing clinical support 
  • Evaluate patients for appropriate GLP-1 therapy and prescribe medications within scope of practice 
  • Monitor treatment response, side effects, laboratory findings, and overall patient progress 
  • Collaborate closely with referring Primary Care Providers and Sylvan Registered Dietitians to coordinate individualized care plans 
  • Educate patients on medication expectations, safety, lifestyle changes, and long-term weight management 
  • Maintain timely and accurate documentation within electronic health record systems 
  • Follow Sylvan clinical standards and partner-specific workflows 
  • Participate in recurring team meetings and collaborate on program development and quality improvement initiatives 
  • Contribute to the ongoing development of Sylvan's medical weight management program and clinical protocols 

Productivity Expectations 

  • Conduct a combination of 40-minute initial visits and 20-minute follow-up visits 
  • Maintain a consistent patient panel and appointment schedule 
  • Meetings, administrative responsibilities, and no-show/late-cancel time are incorporated into the salaried structure 
  • Deliver high-quality, patient-centered care while supporting access and continuity across partner organizations 

Requirements

  • Current Nurse Practitioner certification (FNP, AGNP, or Adult-Gerontology Primary Care NP preferred) 
  • Active, unrestricted Nurse Practitioner license in applicable states or willingness to obtain additional state licensure 
  • DEA registration (or ability to obtain) 
  • Experience managing patients with obesity or metabolic disease 
  • Knowledge of GLP-1 receptor agonists and other anti-obesity medications 
  • Experience with medication titration, monitoring, and patient education 
  • Strong clinical judgment and evidence-based decision-making skills 
  • Excellent communication and patient counseling abilities 
  • Comfortable working independently in a remote telehealth environment 
  • Comfortable using electronic health records and telehealth platforms 
  • Collaborative mindset with enthusiasm for working alongside referring Primary Care Providers and Registered Dietitians in an interdisciplinary care model 
  • Flexibility and interest in helping build new clinical programs and workflows
