Job Summary

We are seeking a Remote House Calls Nurse Practitioner in NJ, OH, PA, NE, TX to support our in-home medical services program. This role provides virtual clinical oversight, patient assessments, care coordination, and provider support for patients receiving care in their homes. The ideal candidate is comfortable practicing telehealth, managing complex chronic conditions, and collaborating with field-based clinicians. 

This is a fully remote position with a strong focus on quality, efficiency, and patient-centered care. 

Responsibilities
  • Conduct telehealth visits for patients enrolled in the House Calls program 
  • Manage chronic conditions such as diabetes, hypertension, CHF, COPD, and geriatric syndromes if the patient requires primary care services 
  • Perform care management needs assessments, follow-ups, and care plan adjustments 
  • Review patient histories, labs, imaging, and diagnostic results 
  • Support medication management and reconciliation 
  • Coordinate care with primary care providers (if patients already have one), specialists, and care management teams 
  • Document visits and clinical decisions accurately in the EMR 
  • Ensure compliance with clinical protocols, quality standards, and regulatory requirements 
  • Participate in interdisciplinary case reviews and quality improvement initiatives 
Qualifications
  • Active Nurse Practitioner (NP) license in the required state(s) 
  • National NP certification (ANCC or AANP) 
  • Minimum 2+ years of clinical experience as a Nurse Practitioner 
  • Experience in house calls, home health, geriatrics, primary care, or internal medicine strongly preferred 
  • Prior telehealth or remote care experience preferred 
  • Strong clinical judgment and comfort managing complex patients remotely 
  • Excellent communication and documentation skills 
  • Must be licensed in NY and one of the other states listed or willing to obtain

Technical & Regulatory Requirements 

  • Proficiency with EMR/EHR systems 
  • Comfortable using telehealth platforms and remote monitoring tools 
  • Active DEA registration (or willingness to obtain, if applicable)
