• Initiate and execute discharge planning on the day of patient admission, laying the groundwork for a successful transition out of the Detox Center.
• Conduct thorough discharge sessions that encompass coping strategies, transition plans, and community resources, guaranteeing patients understand their next steps.
• Confirm discharge appointments with patients and their loved ones, ensuring consistent follow-up and accountability.
• Collaborate closely with referral sources and outpatient service providers to maintain continuity of care, enhancing overall treatment effectiveness.
• Facilitate transfers for patients needing higher levels of care, proactively assessing their situation and urgency.
• Engage in clinical outreach and program tours to promote our services within the community.
• Manage crises in therapy sessions and throughout the treatment milieu effectively, employing intervention strategies to promote safety and well-being.
• Support milieu staff by training them in best practices for patient interactions and maintaining a therapeutic environment.
• Work in partnership with the treatment team and milieu staff to ensure the highest standards of clinical quality and efficacy in patient care.
• Perform assessments for suicidality and threats to self or others, ensuring appropriate interventions are taken promptly.
• Provide auxiliary clinical support to other team members in their absence to promote fluidity and teamwork.
• Communicate with the Utilization Review and Business Departments concerning patient authorizations and billing processes.

The ideal candidate will work with a diverse client population, including individuals experiencing substance use disorders, mental health challenges, and related psychosocial issues. This role necessitates a deep understanding of the various stressors affecting patients in these vulnerable states, as well as a commitment to cultural competency and inclusivity in treatment approaches.

• Master's degree in Social Work, Counseling, Psychology, or a related mental health field.
• Must possess an independent license as a Licensed Clinical Social Worker (LCSW) in the state of California.
• Exemplary documentation skills, including the timely completion of initial assessments, SMART treatment plans, and clinical case notes.
• Proficiency in utilizing Office 365 and Electronic Health Record systems to document services and manage patient data effectively.
• Strong written and verbal communication skills to facilitate effective interactions with patients, families, and multidisciplinary team members.
• Demonstrated ability to establish and maintain appropriate professional boundaries with clients, fostering a trusting therapeutic relationship.
• Experience working with a diverse clientele, ensuring all interactions are respectful and relevant to individual backgrounds.

• Prior experience in a detox or substance use treatment facility.
• Experience with crisis intervention and management in clinical settings.
• Familiarity with treatment planning and follow-up strategies in a behavioral health context.
