This position allows for remote work, granting you the flexibility to manage your workload while accessing vital resources and support from our centrally located hospice center in Knoxville, Tennessee. As part of your role, you will utilize telehealth platforms to conduct therapy sessions, ensuring that our clients receive uninterrupted care in the comfort of their own environments.

• Conduct thorough client assessments to identify individual needs and develop personalized, evidence-based treatment plans tailored to address mental health issues associated with terminal illness and end-of-life care.
• Facilitate individual, couples, and family therapy sessions to help clients navigate their emotional responses and develop coping strategies. Techniques may include cognitive-behavioral therapy, mindfulness, and other therapeutic modalities.
• Collaborate closely with interdisciplinary teams, including physicians, nurses, and chaplains, to contribute to comprehensive patient care, ensuring that all aspects of patient well-being are addressed.
• Maintain meticulous and up-to-date patient records and progress notes, ensuring compliance with all legal and ethical standards.
• Engage in regular supervision and professional development activities to enhance clinical skills and remain informed about best practices in the field.
• Contribute to the organization by participating in team meetings to discuss case management and collaborate on patient care strategies.

The primary focus of this role will be working with patients in hospice care, who may be dealing with terminal illnesses and the emotional complexities of end-of-life circumstances. Your expertise will be vital in providing emotional support and guidance to patients and families, helping them find comfort and resilience during their most difficult moments.

• A Master’s degree in Social Work from an accredited program is required.
• An active state license as a Licensed Clinical Social Worker (LCSW) in Tennessee is essential for this role.
• A minimum of three years of experience working in a mental health or outpatient setting, with specific experience in hospice or palliative care preferred.
• A strong understanding of various evidence-based therapeutic modalities and treatment approaches suited for clients facing chronic illness is essential.
• Exceptional interpersonal, communication, and assessment skills to develop rapport with clients and collaborate effectively with healthcare professionals.

• Experience with trauma-informed care and addressing grief and loss issues.
• Familiarity with teletherapy platforms and electronic health record systems to facilitate remote client engagements.
• Previous involvement in interdisciplinary teams within healthcare settings.

We offer a competitive salary ranging from $80,000 to $100,000 annually, commensurate with experience and qualifications.
