As a Licensed Clinical Social Worker at NovaCare, you will play a critical role in delivering comprehensive psychotherapy services. Your key responsibilities will include:

• Conducting detailed clinical assessments to understand each patient’s unique psychological and social circumstances.
• Developing personalized treatment plans using evidence-based practices that align with the specific needs of individuals and their families facing end-of-life issues.
• Utilizing a variety of therapeutic modalities, such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT), to effectively address diverse mental health concerns.
• Maintaining accurate clinical documentation, ensuring compliance with HIPAA and other relevant regulations.
• Collaborating closely with a multidisciplinary treatment team, including physicians, nurses, and other mental health professionals, to provide holistic and coordinated patient care.
• Engaging in regular case consultations and peer review meetings to foster continuous improvement in service delivery.
• Educating patients and their families on mental health issues, treatment options, and community resources available to support their journey.
• Staying informed with ongoing research and developments in the mental health field to enhance your clinical practice.

Your work will primarily involve supporting individuals and families who are experiencing significant emotional and psychological challenges related to hospice care. This patient population may include those coping with terminal illnesses, grief, anxiety, and depression. Your empathetic nature and clinical skills will be vital in helping them navigate these complexities.

• A Master's degree in Social Work from an accredited institution.
• A valid license to practice as a Licensed Clinical Social Worker (LCSW) in the state of Colorado.
• Minimum of two years of clinical experience, particularly in providing psychotherapy services.
• Demonstrated proficiency in solution-focused therapy techniques.
• Exceptional communication and interpersonal skills, enabling effective interactions with patients, families, and team members.
• Strong analytical and problem-solving skills to assess and respond to patient needs effectively.

• Experience in a hospice or palliative care setting is highly desirable.
• Familiarity with telehealth systems and platforms to facilitate remote patient care.
• A passion for working with vulnerable populations and providing compassionate support during critical life transitions.
