Are you ready to make a significant impact on patients' lives through expert nutrition counseling? At True Rx Health Strategists, we're looking for a dedicated Clinical Dietitian to join our team and help us transform healthcare through comprehensive, value-based nutritional care.

The Clinical Dietitian is responsible for performing a variety of specialized duties related to the nutrition counseling of patients / families for Medical Nutrition Therapy (MNT) diet education for complex medical conditions such as diabetes, obesity, and cardiovascular disease. The primary emphasis isย to serve our clients and members with comprehensive, value based, nutritional care to achieve definitive outcomes that improve a patient's quality of life.ย

What You'll Do:

  • One-on-One Counseling: Provide extensive nutritional and lifestyle education to patients via telehealth to enhance their health and reduce healthcare costs.
  • Sustainable Weight Loss: Work closely with patients to achieve and maintain healthy weight loss.
  • Resource Education: Guide patients to appropriate community resources to support their health journey.
  • Outcome Evaluation: Assess nutritional and lifestyle outcomes, developing impactful reports to track progress.
  • Clinical Recommendations: Formulate and communicate clinical recommendations and visit summaries to healthcare providers and patients.
  • Barrier Identification: Proactively identify and address barriers to effective patient care.
  • Collaborative Development: Partner with departments to leverage technology and health information for better patient outcomes.
  • Client Reporting: Deliver clinical reports and presentations in partnership with the account manager, educating clients on their benefit plans.
  • Program Implementation: Educate clients on clinical programs and solutions to increase adoption and implementation.
  • Team Leadership: Plan, organize, and direct responsibilities as delegated by the Clinical Innovation Pharmacist.

What You Bring:

  • Educational Background: Bachelorโ€™s degree required; Registered Dietitian Certification (CDR); State Certification or Licensure.
  • Experience: Strong work experience counseling individuals in an outpatient setting with complex disease states to prevent complications.
  • Knowledgeable: Familiarity with current treatment standards and options for common metabolic conditions, including diabetes and obesity.
  • Communicator: Excellent verbal and written communication skills.
  • Tech-Savvy: Proficiency in MS Word, Excel, and Outlook, with experience creating and updating Excel spreadsheets.
  • Independent Worker: Ability to work independently with minimal supervision.
  • Patient Advocate: Skilled in identifying patient diet/lifestyle barriers and developing strategies to overcome them.
  • Motivational Interviewing: Significant experience in patient education and engagement.
  • Willing to Travel: Travel as required.
