• Conduct comprehensive bio-psychosocial evaluations to identify the emotional, social, and environmental factors affecting patient health.
• Develop, implement, and assess personalized treatment plans utilizing evidence-based interventions tailored to each patient's unique needs.
• Provide individual and group therapy to address a variety of psychosocial challenges, facilitating discussions that promote healing and empowerment.
• Collaborate with medical staff and a multidisciplinary team to develop integrated care strategies that enhance treatment compliance and improve patient outcomes.
• Document clinical activities in accordance with departmental standards, ensuring accurate representation of patient interactions and outcomes.
• Offer education and resources to patients and their families for understanding treatment recommendations and enhancing acceptance of care plans.
• Coordinate advocacy efforts within the community, focusing on connecting patients with crucial social support and mental health resources.
• Stay updated on industry best practices and participate in ongoing professional development opportunities to enhance clinical skills.

This role will focus on providing mental health services to individuals in hospice care, often dealing with complex emotional challenges related to serious illness and end-of-life issues. As a member of our team, you will be instrumental in helping patients and their families navigate grief, loss, and transition by employing therapeutic techniques that respect their personal journeys and cultural backgrounds.

• A Master’s Degree in Social Work from an accredited graduate school of Social Work.
• Current and active LCSW license from the State Board of Social Work Licensure in Maryland.
• Strong knowledge of clinical practices and social work ethics as guided by the National Association of Social Workers (NASW) Code of Ethics.
• Demonstrated ability to provide culturally competent care and employ an understanding of developmental stages throughout the care process.

• At least 6 months to 1 year of clinical experience in a healthcare or mental health setting, particularly with end-of-life care.
• Familiarity with community resources and ability to advocate for patients’ needs effectively.
• Experience with interdisciplinary collaboration to enhance patient care and outcomes.
