What is the Intake Nurse role?

This position is for an Intake Nurse to be a part of providing outstanding care for members with diseases such as hypertension and diabetes through remote patient monitoring. The Intake Nurse will be the face of our Nursing Team. They will be responsible for establishing credibility and trust with the member, and providing a pleasant and informative first impression, while gathering pertinent medical information from each new member.
This position offers full-time hours (Monday through Friday 8:30am-5pm local time), benefits, and the ability to work from home.

What you'll be doing:

  • Provide a welcoming and informative experience for all new members during their enrollment process.
  • Ensure all pertinent medical information (including medication list and medical history) is collected accurately.
  • Assist our Provider Team in the assessment and enrollment of new members.
  • Accurately and confidently educate new members on the details and benefits of the program
  • Ensure members are informed on the expectations of device usage.
  • Order remote monitoring devices for member
  • Ability to perform high quality, high volume calls
  • Maintain a culture of kindness, teamwork, compassion, and mutual respect

What you'll bring

  • Must have a current, active, unrestricted LPN multi-state compact license in a nursing compact state and be in good standing with the Board of Registered Nurses.
  • A minimum of 2 years hands on nursing experience is required; telehealth and chronic care management experience is strongly preferred.
  • EHR experience, specifically AthenaHealth, is a plus.
  • Fluency in English and Spanish is highly preferred.
  • Comfortable working on a computer; experience navigating through multiple computer applications while on calls; Macbook experience is a plus.
  • Must have reliable high-speed internet.
  • Strong time management skills and self-motivation.
  • Must be a quick learner in a fast paced, changing environment.
  • Professional and effective communication style that translates across all members of the team and members.
  • An attitude and work ethic that meets the core values and goals of CopilotIQ.
