This role is designed to be fully remote, allowing the LCSW substantial flexibility while ensuring consistent support to students in school settings. You will be embedded within our multidisciplinary rehabilitation teams, collaborating closely with physical therapists, occupational therapists, and speech-language pathologists. Our goal is to create a cohesive support system that addresses not only the clinical needs of our patients but also their emotional and social challenges.

• Conduct comprehensive psychosocial assessments through detailed interviews and analysis of medical records, identifying emotional, social, and environmental factors impacting students' recovery.
• Provide focused counseling and support services to students and their families, skillfully addressing adjustment issues and promoting emotional health throughout their rehabilitation journey.
• Collaborate closely with the rehabilitation team to design and implement individualized treatment plans that include consideration of students' psychosocial needs.
• Assist students in navigating community resources such as support groups, vocational rehabilitation, and housing assistance to enhance their access to necessary services.
• Deliver educational resources to patients and families about community support and post-rehabilitation services, fostering a better understanding of available options.
• Engage in proactive discharge planning, guiding students through transitions back to their home or other supportive environments.

The LCSW will work primarily with school-aged children and adolescents within various educational settings, helping them to address the psychological impacts of injuries, illnesses, and disabilities. By focusing on the emotional and social dimensions of their battles, our LCSW plays a critical role in fostering their successful reintegration into school systems and communities.

• Master's degree in Social Work from an accredited program is required.
• An active state license as a Licensed Clinical Social Worker (LCSW) is mandatory to ensure compliance with professional standards.

• Prior experience working in a rehabilitation or medical setting is preferred, highlighting familiarity with interdisciplinary collaborations.
• A strong understanding of adjustment disorders, grief, and coping strategies will benefit both the position and the clientele.
