• Conduct comprehensive psychosocial assessments through in-depth interviews, direct observations, and analysis of medical records, identifying key emotional, social, and environmental factors influencing patient recovery.
• Deliver individualized counseling and emotional support to patients and their families, helping to navigate adjustment challenges and bolster their emotional resilience during the rehabilitation journey.
• Collaborate closely with cross-functional rehabilitation teams to develop tailored treatment plans that incorporate patients' psychosocial needs and leverage community resources effectively.
• Guide patients in accessing community services, including support groups, vocational rehabilitation, housing assistance, and continuation of care post-rehabilitation.
• Participate in discharge planning efforts, facilitating transitions that ensure patients can leave the rehabilitation setting smoothly and continue their road to recovery.

In this role, you will work with a diverse patient population, including individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. Your expertise will play a vital role within our interdisciplinary team as you help patients overcome emotional and social barriers, enabling them to achieve their maximum potential for functional independence.

• Master's degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Proven ability to conduct thorough assessments and effectively navigate complex social issues impacting patient care.
• Outstanding communication and interpersonal skills, essential for building rapport with patients, families, and team members.

• Prior experience in a rehabilitation or medical setting is preferred, showcasing an understanding of transitional care and rehabilitation processes.
• Knowledge of grief, adjustment issues, and effective coping strategies to support patients through difficult times.
