• Conduct Psychosocial Assessments: Perform thorough evaluations by interviewing patients, observing their interactions, and reviewing their medical histories to determine social and emotional factors affecting rehabilitation.
• Provide Counseling: Offer robust therapeutic support to patients and their families, aiming to address adjustment issues and manage emotional well-being throughout the rehabilitation process.
• Develop Treatment Plans: Collaborate with the rehabilitation team to create individualized treatment plans that account for the unique psychosocial needs of each patient, ensuring a holistic approach to care.
• Facilitate Resource Access: Assist patients in navigating community resources such as support groups, vocational rehabilitation, or housing assistance, ensuring a smooth transition from rehabilitation back into the community.
• Participate in Discharge Planning: Engage actively in discharge planning processes, ensuring that patients have the necessary support and resources as they prepare to transition to home or other care settings.

As a Licensed Clinical Social Worker, you will work predominantly with patients in rehabilitation for various conditions including but not limited to strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Your role will be vital in addressing the emotional and psychological barriers these patients face, enhancing their ability to reintegrate into their communities and lead fulfilling lives.

• Educational Requirement: A Master's degree in Social Work from an accredited program is required.
• Licensing: An active state licensure as a Licensed Clinical Social Worker (LCSW) is essential for this position.

• Experience in a rehabilitation or medical setting, understanding the unique challenges faced by patients undergoing physical recovery.
• A strong grasp of adjustment issues, coping strategies, and grief processes to effectively support patients in their rehabilitation journeys.
• Exceptional communication and interpersonal skills, with a proven ability to build rapport with clients and families.

The offered salary range for this full-time position is between USD 75,000 and 100,000 annually, reflective of your experience and qualifications.
