• Conduct comprehensive psychosocial assessments through patient interviews, medical record evaluations, and observational methods, identifying emotional, social, and environmental factors that may affect recovery.
• Provide empathetic counseling and emotional support to patients and their families, addressing various adjustment issues and enhancing overall mental wellness.
• Collaborate with the interdisciplinary rehabilitation team to develop and implement individualized treatment plans that reflect the psychosocial needs and preferences of each patient.
• Assist patients in navigating community resources, such as support groups, vocational rehabilitation services, and housing assistance programs, empowering them during their recovery process.
• Educate families about the available community resources and post-rehabilitation services to ensure a smooth transition and sustained support post-discharge.
• Engage in thorough discharge planning and facilitate seamless transitions to home care or alternative settings, ensuring continued patient support.

• A Master’s degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).

• Exceptional understanding of psychosocial dynamics, adjustment disorders, grief, and coping mechanisms.

• Previous experience in a rehabilitation or medical setting is highly preferred, particularly in environments involving complex patient needs.
• Familiarity with community resources and social service systems relevant to patient rehabilitation.
