• Conduct thorough psychosocial assessments, utilizing interviews, observations, and reviews of medical records, to evaluate the emotional, social, and environmental factors that impact patients' recovery.
• Provide empathetic counseling and support to patients and their families, helping them adjust to new realities and providing strategies for emotional well-being.
• Work collaboratively with the rehabilitation team, including physical and occupational therapists, to develop and implement individualized treatment plans that address patients' psychosocial needs.
• Assist patients in navigating community resources, which may include support groups, vocational rehabilitation services, and housing assistance programs.
• Educate patients and families about available community resources and post-rehabilitation services, ensuring they have the tools needed for successful transitions.
• Participate actively in discharge planning efforts to facilitate smooth transitions from care settings, providing continuity in support and resources.

Your work will directly impact a diverse patient population, including those recovering from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. You will play a crucial role in addressing their emotional and social needs, guiding them towards successful reintegration into their communities. • A Master’s degree in Social Work from an accredited program.
• Active and valid state licensure as a Licensed Clinical Social Worker (LCSW).

• Experience in a rehabilitation or medical setting is highly desirable, providing a foundation for understanding the challenges faced by patients in recovery.
