• Conduct thorough psychosocial assessments through patient interviews and medical record reviews to accurately evaluate the emotional, social, and environmental factors affecting recovery.
• Provide individualized counseling and emotional support to patients and their families, helping them navigate adjustment issues related to their conditions.
• Work closely with the rehabilitation team to create and implement personalized treatment plans that address the psychosocial needs of each patient.
• Guide patients in accessing community resources—including support groups, vocational rehabilitation, and housing assistance—to aid in their recovery process.
• Educate patients and families about available community resources and post-rehabilitation services, ensuring a smooth transition back to everyday life.
• Actively participate in discharge planning and coordinate the transition of patients from rehabilitation to home or into other care settings as necessary.

As an LCSW at Beyond the Edge Mental Health Clinic, you will support a wide array of patients, including those recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Our patients present with unique challenges that require a nuanced understanding of emotional and social dynamics, which will be critical in enhancing their recovery experiences.

• A Master’s degree in Social Work from an accredited institution.
• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong interpersonal and communication skills to effectively engage with patients and their families.
• A robust understanding of adjustment issues, grief, and coping mechanisms as they relate to patient outcomes.

• Previous experience working in rehabilitation or healthcare settings where psychosocial support was necessary.
• Familiarity with community resources available for patient support post-discharge.
• Experience participating in interdisciplinary treatment team meetings to maximize patient care efforts.

The Licensed Clinical Social Worker (LCSW) position offers a competitive annual salary ranging from $75,000 to $100,000, along with a sign-on bonus.
