• Conduct comprehensive psychosocial assessments through patient interviews, direct observations, and review of medical records, identifying emotional, social, and environmental factors affecting recovery.
• Deliver therapeutic counseling and emotional support to patients and their families, focusing on adjustment challenges and mental wellness.
• Collaborate with an interdisciplinary rehabilitation team to craft personalized treatment strategies that incorporate the psychosocial aspects of care.
• Assist patients in accessing vital community resources such as vocational rehabilitation, housing assistance, and support groups to enhance their recovery journey.
• Provide education to patients and families on available post-rehabilitation services and community resources to encourage ongoing support.
• Play a key role in discharge planning, ensuring smooth transitions for patients returning home or moving to alternative care settings.

Our clinic serves a diverse patient population, including individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. You will be pivotal in addressing not just clinical needs, but also the emotional and social challenges that can accompany recovery, thus fostering a comprehensive path towards independence.

• Master's degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong interpersonal and communication skills, with the ability to engage with patients empathetically and effectively.
• A solid understanding of adjustment challenges, grief processes, and coping strategies necessary for rehabilitative success.

• Previous experience in a rehabilitation or medical setting is highly preferred, providing a strong foundation in understanding the complexities of patient recovery.
