• Conduct comprehensive clinical assessments to evaluate patients’ mental health needs and emotional well-being.
• Develop individualized treatment plans that incorporate evidence-based practices and therapeutic interventions.
• Implement solution-focused therapy approaches to assist patients and their families in coping with end-of-life scenarios.
• Maintain accurate and timely clinical documentation, ensuring compliance with hospice standards and protocols.
• Collaborate effectively with a multidisciplinary care team, communicating regularly to coordinate care plans and interventions.
• Provide psychoeducation and support to patients and their families, helping them navigate emotional challenges associated with terminal illness.
• Stay abreast of the latest research and trends in mental health practices to ensure the delivery of high-quality care.

As an LCSW within our hospice center, you will work closely with a diverse population of patients facing life-limiting illnesses. This role focuses on providing emotional and psychosocial support to these individuals, along with their families, throughout their journey. You will facilitate conversations regarding end-of-life care and help patients navigate complex feelings related to grief, anxiety, and loss.

• A Master’s Degree in Social Work from an accredited institution is required.
• Possession of a valid LCSW license in the state of Texas is mandatory.
• Proficiency in solution-based psychotherapy interventions, particularly in a healthcare setting.
• Exceptional communication, empathy, and problem-solving skills to effectively engage with patients from varied backgrounds.

• Previous experience in a hospice or palliative care setting is highly regarded.
• Familiarity with trauma-informed care practices to address diverse patient needs.
• Demonstrated ability to work collaboratively with healthcare professionals in a team-oriented environment.
