Job description

MaxHealth is looking to hire afull-time Oncology Care Coordinator who will serve as a central point of contact for patients, families, primary care providers, oncologists, surgeons, and other healthcare professionals. This role is responsible for coordinating oncology-related referrals, facilitating timely access to care, monitoring patient progress, and ensuring effective communication among all members of the care team.

This is a remote position but candidates must reside in Florida and have an active Florida Nurse Practitioner license to be considered for this position.

Salary to align with experience.


Responsibilities:

  • Monitor and manage oncology, biopsy, and surgical referrals received from PCP offices.
  • Review and appropriately direct oncology referrals to the appropriate oncologist based on the patient's diagnosis, clinical needs, and physician availability.
  • Coordinate appointments and care between the patient, PCP, oncology office, surgeons, and other healthcare providers.
  • Coordinate biopsy and surgical appointments and ensure that required medical records, imaging, pathology reports, and other clinical information are available to the appropriate providers.
  • Review referrals, imaging reports and historical reports ordered by PCPs or specialists to determine whether an oncology consultation or additional evaluation is indicated.
  • Work closely with the oncology care team, oncologists, primary care, specialists and other clinical leaders to evaluate cases to determine appropriate care pathway.
  • Serve as a consistent point of contact and patient advocate throughout the referral and care coordination process.
  • Monitor patient progress and maintain communication with the referring provider regarding the patient's status, appointments, and ongoing care needs.
  • Follow up with patients to ensure they are attending scheduled appointments and completing recommended testing, biopsies, consultations, and treatments.
  • Conduct follow-up calls with patients to identify concerns, changes in condition, barriers to care, or social needs, and communicate relevant information to the appropriate physician or healthcare provider.
  • Identify and address barriers that may delay or prevent patients from receiving timely oncology care, including scheduling, communication, and coordination issues.
  • Maintain accurate and timely documentation of referrals, patient communications, appointments, follow-ups, and care coordination activities.
  • Track multiple patient cases simultaneously and prioritize tasks effectively to ensure timely follow-up and continuity of care.
  • Coordinate care through interdisciplinary collaboration with physicians, nurses, medical assistants, PCP offices, surgeons, imaging centers, and other healthcare providers.
  • Facilitate communication among members of the care team to promote efficient care coordination and optimal patient outcomes.
  • Provide compassionate, professional, and patient-centered service throughout the patient's care journey.
  • Assist with community outreach efforts.
  • Develop and adhere to quality of care metrics.
  • Perform other duties related to oncology care coordination as assigned.
