Keck Health Care LLC is seeking a dedicated and compassionate Licensed Independent Clinical Social Worker (LICSW) to join our team in a full-time remote capacity. This position operates in a remote setting, providing flexibility and supporting work-life balance while engaging with clients who are in need of emotional and practical social support. The role entails regular collaboration with multidisciplinary teams, including medical personnel and spiritual care providers within our hospice framework, in order to foster an environment of comprehensive patient care. You will need to occasionally adjust to the urgency of client needs, which may require evening and weekend hours, driven by the nature of the hospice care environment.

• Conduct thorough assessments to identify client needs, focusing on the emotional, social, and environmental factors impacting their well-being.
• Develop and implement personalized service plans that address client goals and outline necessary interventions.
• Provide individual and family counseling sessions, facilitating discussions that help clients express their feelings about illness, loss, and grief.
• Coordinate access to essential community resources, including but not limited to financial assistance programs, legal aid, and psychological support services.
• Offer immediate crisis intervention and risk assessments, ensuring clients have safety plans in place when required.
• Work collaboratively with healthcare providers and community partners to create a multidisciplinary approach to patient care, ensuring the client’s holistic needs are met.
• Engage in community outreach initiatives, promoting the services of the hospice center and addressing awareness around end-of-life issues.
• Participate in team meetings and continuing education training to stay informed on best practices in social work and hospice care.

The client population served by this role primarily includes individuals nearing the end of life, along with their families, who are coping with the challenges associated with terminal illness. Every interaction aims to provide compassionate support, improve quality of life, and navigate complex emotions related to death and dying, ensuring that both patients and families feel heard, supported, and empowered.

• Master's degree in Social Work from an accredited program.
• Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in Washington state.
• Solid understanding of hospice care dynamics, end-of-life issues, and the emotional processes associated with grief and loss.
• Exceptional interpersonal and communication skills, with a proven ability to engage effectively with diverse populations and collaborate with interdisciplinary teams.
• Proficient in a range of counseling techniques and case management processes, capable of adapting to the unique needs of clients.

• Previous experience working in a hospice setting or with terminally ill patients is highly desirable.
• Familiarity with trauma-informed care practices and the ability to address social and economic justice issues.
• Experience in crisis intervention and knowledge of community resources pertinent to end-of-life care.

This full-time role offers a competitive salary ranging from $60,000 to $100,000 annually, commensurate with experience. In addition to base salary, employees may be eligible for a sign-on bonus.
