About the Role:
Care Essentials Home Health is looking for a compassionate and dedicated Case Manager to join our growing team in San Bernardino. In this role, you'll make a meaningful difference in patients' lives by coordinating high-quality home health services that support recovery, independence, and well-being. Responsibilities:
- Assess, plan, and coordinate individualized care plans for home health patients
- Serve as the primary point of contact between patients, families, and multidisciplinary care teams
- Monitor patient progress and adjust care plans to meet evolving clinical needs
- Coordinate referrals, authorizations, and community resources to ensure continuity of care
- Conduct home visits and telephonic follow-ups to evaluate patient outcomes
- Maintain accurate and timely documentation in compliance with regulatory standards
- Collaborate with physicians, nurses, and therapists to deliver integrated patient care
- Active RN, LPN, or relevant clinical licensure preferred
- Prior experience in case management, home health, or a related healthcare setting
- Strong knowledge of care coordination, discharge planning, and community resources
- Excellent communication, organizational, and problem-solving skills
- Proficiency with electronic health records (EHR) and documentation systems
- Ability to work independently and manage a diverse patient caseload
- Compassionate, patient-centered approach to care delivery
