What you'll do

Reporting to the Director of Nursing, the Nurse, Remote Patient Monitoring (RPM) and Community Outreach provides remote patient monitoring services and community-based nursing support to our patients. This role is responsible for independently monitoring and analyzing patient data through a secure dashboard and applying clinical judgment to provide timely feedback, education and escalation when appropriate within healthcare and community settings.

The Nurse RPM and Community Outreach will liaise with patients and healthcare providers to ensure that insights from the monitoring platform are effectively integrated into decisions to optimize the patient's care. While practicing with a high degree of autonomy in monitoring and clinical-decision making, the nurse will collaborate with providers, clinic teams, and community partners to support coordinated care when needed.

This role provides specialized nursing services focused on lower-extremity examinations, including sensation testing and skin and wound assessment, with an emphasis on wound prevention and promoting overall skin integrity.

This position will be multi-disciplinary, adapting in the early-stage company environment as needs and as priorities change.

Responsibilities include:

  • Facilitating community health programs and group education
  • Monitoring and analyzing physiological / biometric data as supplemental support to the healthcare providers patient care plan
  • Provide specialized nursing services focused on lower-extremity assessments, including sensation testing and evaluation of overall skin integrity and wound evaluation in community settings across Alberta
  • Liaising between patient and provider to help with patient compliance to their remote monitoring care plan including working with patients and providers to incorporate remote patient monitoring into the continuum of care
  • Making proactive outbound calls to provide clinical support and patient education that is responsive to individual preferences, learning styles, and diverse cultural backgrounds
  • Maintaining accurate records and documenting monitoring actions and discussions in a cloud-based dashboard
  • Traveling to communities to develop relationships and provide patient education, perform physical assessments of the lower limb, dispense insoles, and collaborate with existing care teams to support care coordination as appropriate
  • Ensuring clear and consistent communication with healthcare providers and patients
  • Addressing incoming calls requiring clinical guidance
  • Collaborating as a key stakeholder with other departments (Sales, Marketing, Customer Care, Clinical Affairs, R&D etc.) to optimize the overall patient and provider experience
  • Providing feedback on procedures, policies, and standards
  • Maintaining a clean, and safe working environment
  • Performing duties in a manner that is consistent with and committed to upholding the requirements of the quality management system and professional standards of practice

Additionally, due to the ever-changing and sometimes chaotic environment of an early-stage high-tech company, the Nurse RPM and Community Outreach may assume additional responsibilities, as required.

What you'll bring:

Required Qualifications:

  • Bachelor's degree in nursing (BN/BScN) or Diploma in Practical Nursing
  • Registration and membership in good standing with the College of Registered Nurses of Alberta (CRNA) or the College of Licensed Practical Nurses of Alberta (CLPNA)
  • Clear Vulnerable Sector Check valid within the past year
  • Minimum 5+ years of clinical nursing experience with direct patient care, preferably including lower limb assessment, wound care, vascular or diabetes-related care (remote or in person)
  • Demonstrated competency performing comprehensive lower extremity assessments, including vascular and neurological screening (e.g., monofilament testing for Loss of Protective Sensation)
  • Certification or training in Advanced or Basic Foot Care Nursing, wound care or lower limb / foot health assessment
  • Experience providing patient education and clinical support, with the ability to tailor communication to individual learning styles and needs
  • Strong interpersonal and communication skills with a demonstrated ability to build rapport and maintain therapeutic relationships, both in-person and remotely
  • Ability to analyze patient monitoring data and apply sound clinical judgment to support patient care and escalation when appropriate
  • Proficiency with Microsoft Office (Including Word, Excel, SharePoint and Teams)
  • Proficiency with digital documentation systems / electronic health records
  • Ability to travel within Alberta, primarily supporting clinics in Calgary, with occasional travel to Edmonton and other communities as required

Preferred Qualifications

  • Experience working in wound care clinics, diabetes clinics, vascular care, or community based chronic disease management programs
  • Certification or additional training such as NSWOC (Nurses Specialized in Wound, Ostomy and Continence) or other recognized wound care education
  • Experience with remote patient monitoring or virtual care delivery models
  • Experience working with First Nations, Metis, and Indigenous communities
  • Understanding of social determinants of health and their impact on health outcomes, particularly in rural, remote and Indigenous communities
  • Experience collaborating with interdisciplinary healthcare teams to support coordinated patient care
  • Familiarity with quality management systems (ISO 13485) and HIPAA/PIPEDA standards
  • Comfortable using digital health technologies and performing basic technical troubleshooting across smartphone platforms and remote monitoring tools

The details

Employment status: Temporary full-time, 40 hours per week for 12 months. This role has the possibility to become permanent full time as the business scales.

Schedule:40 hours per week, Monday to Friday, with a variable schedule between 8:30 a.m. and 5:00 p.m. Daily hours may vary based on patient requirements and community outreach events. The incumbent must be able to flex their schedule as needed to accommodate these responsibilities. There may be times when additional hours are required.

Work location:Hybrid, Calgary Alberta. Please note this is a hybrid position and this position is required to work onsite at our Head Office located at Suite 205, 1240 - 20thAvenue S.E. as the business requires. This position is required to travel to locations throughout Alberta.
