Job description

Form Health is a virtual obesity medicine clinic delivering multi-disciplinary evidence-based obesity treatment through telemedicine. Obesity impacts more than 40% of the US adult population, and although historically only about 1% of patients received medical treatment for their disease, the field of Obesity Medicine is entering a period of rapid growth. Form Health provides high-quality expert care and leverages technology to enhance the patient experience. All Form Health patients work closely with their physician-led care team, which also includes advanced practice professionals and Registered Dietitians. Through our proprietary mobile app patients engage in regular video visits, as well as text messaging, photo journaling, digital data transmission, and customized educational materials. We hold ourselves to the highest standards of clinical care, and to treating every individual with empathy and respect.ย

Founded in 2019, Form Health is a venture-backed innovative startup with an experienced clinical and leadership team. Our mission is to empower patients and be leaders in Obesity Medicine driving impact at a national scale. We are deeply invested in our core value to put patients first, and also deeply committed to creating a culture where every employee is valued and we learn and improve together.ย

About the Role:

This role will include participation in a rotating after hours call schedule with Form Health clinical leadership and will have a reduced clinical schedule the weeks of on-call coverage.

This is a Modified Full Time position based in a 100% remote (must be based in the US), telemedicine clinical setting. The physician will work closely with a supportive care team and have protected time for administrative duties.ย

What you will do:
  • Provide direct patient care in a clinical setting, focusing on evidence-based obesity treatment and chronic weight management. The physician will have their own panel and will collaborate with Registered Dietitians.
  • At this time, this role does not collaborate with APPs. In the future, this physician role will be responsible for chart reviews and conduct return visits for APP patients every third visit to ensure quality and continuity of care.
  • Provide treatment planning and medical management support for complex cases or escalated concerns.
  • Prescribe non-controlled obesity medications for patients under direct care
  • Ensure clinical documentation, protocols, and treatment pathways are followed consistently within the team.
  • Assist in the professional development of APPs by fostering clinical growth, offering feedback, and supporting continuing education. This role could include APP collaboration and supervision responsibilities.ย
  • Participate in team-based care initiatives and contribute to a culture of collaboration and high-quality care.
  • Attend relevant departmental meetings and contribute to quality improvement efforts within the obesity medicine program.

About You:

  • MD or DO degree with board certification; must be an American Board of Obesity Medicine (ABOM) diplomate
  • Are licensed to practice and prescribe legally in ALABAMA, MISSOURI or MISSISSIPPI. Licensure in multiple states is a nice-to-have, but not required
  • This position requires multi-state practice capability. Applicants must either already hold a valid Letter of Qualification (LOQ) through the Interstate Medical Licensure Compact (IMLC) or meet all prerequisites to rapidly secure one upon hire
  • Current or previous credentialing with insurance payers is desired, but not required
  • Have excellent interpersonal skills for patient and team interactions
  • Are committed to the highest quality of medical care and an evidence-based approach to obesity managementย
