• Conduct in-depth assessments of patients’ needs, abilities, and behaviors utilizing various methods such as clinical interviews and direct observation, enabling tailored care plans that address specific patient concerns.
• Implement evidence-based treatment modalities for mental health challenges, including anxiety, depression, end-of-life anxieties, and relationship difficulties, ensuring holistic approaches that incorporate both psychological and social dimensions.
• Serve as a key consultant to healthcare professionals, advocating for integrated care strategies and fostering a culture of compassionate, interdisciplinary treatment that enhances patient outcomes.
• Maintain meticulous and accurate medical documentation in line with state and federal regulations, ensuring compliance and a high standard of care delivery.

In this role, you will primarily work with patients in hospice care who are dealing with serious illnesses, end-of-life issues, and mental health challenges that accompany these circumstances. Your expertise will be pivotal in helping individuals navigate their emotional responses, providing psychological support to both patients and their families, and facilitating discussions around advanced care planning and grief management.

• Master’s degree in Social Work (MSW) from an accredited program.
• Current, active license issued by the State of California as a Licensed Clinical Social Worker (LCSW).
• Minimum of two (2) years of professional experience in a mental health care setting, demonstrating an understanding of psychosocial issues and therapeutic interventions.
• Strong capacity to work with individuals facing psychiatric disabilities and complex emotional issues, showcasing flexibility and adaptability in approaches.

• Experience in hospice and palliative care settings, enhancing understanding of end-of-life issues and the unique needs of terminally ill patients.
• Previous involvement in multidisciplinary teams, emphasizing communication and collaboration across healthcare disciplines.
• Familiarity with telehealth platforms and electronic health records to support remote service delivery.

This role offers a competitive salary range of USD 120,000 – 130,000 annually, commensurate with experience and qualifications.
