• Conduct comprehensive psychosocial assessments that involve patient interviews, thorough observations, and meticulous review of medical records to identify emotional, social, and environmental factors influencing recovery.
• Provide empathetic counseling and support to patients and their families, focusing on addressing issues related to adjustment, emotional hardships, and daily coping strategies.
• Collaborate closely with the rehabilitation team, which includes medical professionals and therapists, to develop individualized treatment plans that meet the unique psychosocial needs of each patient.
• Assist patients in navigating community resources, including support groups, vocational rehabilitation programs, and housing assistance, ensuring a smooth transition post-treatment.
• Educate and empower patients and their families regarding community resources and available post-rehabilitation services to aid in long-term recovery.
• Actively participate in discharge planning, coordinating transitions to home or alternative care settings, thereby promoting successful reintegration into the community.

The primary focus of this role lies in working with patients experiencing substance use disorders, as well as related mental health challenges. By addressing the emotional and social factors that affect recovery, you will play an integral part in their rehabilitation process. Your expertise will assist in creating a supportive atmosphere that respects their experiences and encourages personal growth and community involvement.

• A Master's degree in Social Work from an accredited program.
• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong communication and interpersonal skills that facilitate effective collaboration with patients, families, and healthcare teams.

• A background in a rehabilitation or medical setting, with a focus on mental health and substance use treatments.
• An understanding of adjustment issues, grief, and coping mechanisms that patients may encounter on their path to recovery.
