Key Responsibilities
  • Conduct comprehensive assessments of clients to understand their unique needs and develop individualized treatment plans that align with their goals.
  • Maintain open communication with supervisors, colleagues, and family members to track each client’s progress, providing updates and collaborating on care strategies.
  • Create and share weekly schedules for therapy appointments, home visits, and observations, ensuring a structured approach to each client’s care.
  • Coordinate and execute home visits as necessary to meet in-home observation requirements, facilitate chart reviews, and conduct staff interviews.
  • Ensure that treatment plans are effectively implemented and adjusted in response to clients’ evolving needs.
  • Participate actively in interdisciplinary team meetings, bringing valuable insights and discussing strategies for client care.
  • Complete monthly and quarterly progress notes that accurately document clients’ advancements and challenges.
  • Organize and oversee staff training sessions to ensure everyone is competent in executing the treatment plans and related documentation.
  • Provide ongoing follow-up to clients through interviews and home visits to ensure they have the support they need to flourish.
  • Maintain detailed and organized records, making information accessible to relevant parties while ensuring confidentiality.
  • Act as a liaison within the community, establishing relationships with other mental health professionals and contributing to the wider mental health initiatives.
  • Assist in the development of training materials and procedures aimed at enhancing the quality of programs provided to clients.
Patient Population / Client Focus

The LPC will work with a varied patient population, including both adults and youth, addressing a spectrum of mental health challenges. Counselors will engage with individuals struggling with trauma, anxiety, depression, and stress-related disorders. Qualifications
