Position Overview

Bliss Reformation and Wellness Center is seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our team on a full-time basis in a remote school setting. This pivotal role involves providing tailored psychosocial support and intervention for seniors, empowering them to achieve a higher quality of life. Operating within a collaborative and nurturing environment, you will play a critical role in assessing needs and developing comprehensive care plans to enhance the emotional and mental well-being of our clients.

Work Environment

This position offers a flexible remote work setting while also requiring occasional visits to the client’s residences as part of the uninterrupted care strategy. Working hours are typically from Monday to Friday, 8:00 AM to 4:30 PM, with the understanding that flexibility is vital to accommodate client needs. Key Responsibilities
  • Conduct comprehensive assessments of clients' psychosocial needs, with a focus on understanding emotional, mental, and social factors impacting their daily lives.
  • Develop and implement individualized care plans in collaboration with seniors and their families that prioritize their unique goals and preferences.
  • Provide compassionate counseling and emotional support to clients, guiding them through life transitions such as health changes, loss, or family dynamics.
  • Advocate for clients' rights and effectively connect them with community resources, ensuring they have access to necessary support services.
  • Work closely within interdisciplinary teams, including healthcare professionals, to develop holistic care strategies that improve client outcomes.
  • Perform regular home visits to assess living conditions, monitor well-being, and address any emerging safety concerns.
  • Facilitate open communication among family members surrounding the client’s care to resolve conflicts and enhance collaboration.
  • Educate clients and their families about community resources, including support groups and programs aimed at increasing social engagement.
  • Ensure all client interactions, assessments, and treatment plans are documented accurately within electronic health record (EHR) systems.
  • Keep abreast of latest developments in social work practices and regulations, ensuring compliance with ethical standards to deliver optimal care.
Patient Population / Client Focus

This role will primarily assist seniors facing various emotional and psychological challenges, including issues related to aging. Our approach combines compassion and expertise, focusing on empowering our clients and their families to actively engage in their mental health journey. By addressing individual needs, we aim to restore confidence and promote an improved quality of life.

Qualifications
  • A Master's degree in Social Work from an accredited institution.
  • Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
  • A minimum of two years of experience working in social work, ideally in community centers or settings focused on geriatric care.
  • In-depth knowledge of social work principles, aging population concerns, and available community resources.
  • Exceptional skills in assessment, counseling, and problem resolution.
  • Empathetic communication style with a capacity to build rapport with seniors and their families.
  • Strong interpersonal skills and an ability to collaborate effectively in multidisciplinary teams.
  • Proficiency in the use of electronic health record (EHR) systems and standard computer applications.
Preferred Experience
  • Experience in a school or educational setting that supports seniors and their mental health.
  • Familiarity with Medicaid and Medicare systems participating in care coordination.
