• Lead psychoeducational and process-oriented therapy groups, utilizing evidence-based practices to foster a safe group dynamic.
• Conduct individual and group therapy sessions designed to address specific mental health needs, ensuring that therapeutic techniques align with client goals.
• Perform comprehensive clinical assessments to determine patient needs, strengths, and treatment plans.
• Manage various case management duties, which include collaborating with the Utilization Review and Business Departments to ensure authorization and payment processes are handled efficiently.
• Initiate discharge planning from the moment of admission, ensuring that clients are well-prepared for transition back into their communities via follow-up sessions that solidify coping skills and resources.
• Proactively communicate with referral sources and outpatient providers throughout the treatment process to ensure continuity of care at all stages—admission, midpoint, and discharge.
• Document all interactions, assessments, and service deliveries in a timely manner to maintain updated and accurate client records.

As a Licensed Clinical Social Worker, you will work with a wide variety of clients including individuals experiencing anxiety, depression, trauma, and other mental health conditions.
