We are seeking a dedicated and skilled Clinical Social Worker (CSW) to join our team at BayCare Mental and Behavioral Center in Jacksonville, Florida. This full-time role is focused on providing remote psychological support and counseling to our patients within a school setting. As a Licensed Clinical Social Worker (LCSW), you will help individuals navigate emotional, social, and environmental challenges affecting their rehabilitation and reintegration into the community.

• Conduct thorough psychosocial assessments to evaluate patients’ emotional, social, and environmental factors impacting their recovery journey.
• Provide counseling and emotional support to patients and their families, addressing issues of adjustment, grief, and psychological well-being during their rehabilitation.
• Collaborate closely with the rehabilitation team to develop and implement individualized treatment plans tailored to the unique psychosocial needs of each patient.
• Assist patients in accessing essential community resources, such as vocational rehabilitation services, housing assistance, and support groups, to enhance their recovery.
• Educate patients and their families about available community resources and post-rehabilitation services that foster long-term adjustment.
• Participate proactively in discharge planning and facilitate smooth transitions to home or alternative care settings, ensuring patients are supported post-discharge.

In this role, you will focus on a diverse patient population that includes individuals navigating significant physical and emotional challenges. Patients may be recovering from various conditions such as strokes, spinal cord injuries, orthopedic surgeries, or neurological disorders. Your contributions will directly impact their rehabilitation experiences and ultimately their ability to reintegrate into their communities successfully.

• Master’s degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong understanding of psychosocial issues, adjustment challenges, and coping mechanisms.
• Excellent communication and interpersonal skills, essential for building rapport with patients and their families.

• Previous experience within a rehabilitation or medical setting is highly preferred.
• Familiarity with interdisciplinary collaboration and goal-oriented patient care methodologies.

We offer a competitive salary range of USD 75,000 to 100,000 annually, along with a generous sign-on bonus.
