In this fully remote role based out of Waldorf, Maryland, you will be part of a collaborative and innovative team focused on achieving optimal patient outcomes. Key Responsibilities
  • Conduct ongoing assessments to identify and meet the specific needs of your patients, ensuring a tailored approach to care.
  • Maintain open lines of communication with supervisors, direct care staff, family members, and guardians to discuss client progress and treatment plans.
  • Coordinate and provide a structured schedule of appointments, home visits, and observational sessions to monitor and support patient improvement.
  • Facilitate home visits that involve in-home observations, chart reviews, and staff interviews, ensuring comprehensive assessments are conducted.
  • Oversee the effective implementation of treatment plans, ensuring all team members execute the program with fidelity.
  • Participate actively in interdisciplinary team meetings to review patient care strategies and outcomes.
  • Prepare detailed monthly and quarterly progress notes that reflect the ongoing development and treatment of clients.
  • Organize training sessions to ensure that all staff members are proficient in the program plan and related documentation procedures.
  • Establish follow-up protocols for interviews, home visits, and observations to provide continual support tailored to individual needs.
  • Maintain comprehensive and accurate patient records for easy access by all stakeholders involved in client care.
  • Act as a liaison with the wider mental health community, promoting greater collaboration and development support.
  • Assist in the creation of training materials and procedures aimed at enhancing service delivery and program effectiveness.

Qualifications
