• Conduct thorough initial assessments of patients’ mental health needs, developing treatment plans that align with their unique situations.
• Facilitate effective virtual therapy sessions tailored to the individual’s psychological challenges and personal goals.
• Maintain accurate and confidential patient records within our Simple Practice platform, ensuring compliance with healthcare regulations.
• Collaborate with interdisciplinary teams within the hospice framework, including doctors, nurses, and family members, to coordinate comprehensive patient care.
• Empower patients by providing evidence-based therapeutic interventions and ongoing support as they navigate complex emotions associated with grief and loss.
• Stay informed about the latest mental health trends and practices to continuously enhance the quality of care.

The ideal candidate will work primarily with adult patients facing terminal illnesses, their families, and caregivers. This role requires sensitive and compassionate engagement as patients and their loved ones cope with grief, anxiety, and emotional distress. Your knowledge and understanding of hospice care will enable you to provide the specialized support needed for this vulnerable population.

• A Master's degree in Counseling, Social Work, or Marriage and Family Therapy from an accredited institution.
• Current licensure as a Mental Health Clinician (LMFT, LPC, or LCSW) is essential.
• Superb communication skills, both verbal and written, to foster trust and connection with clients in a virtual setting.
• Strong knowledge and experience in treatment planning and mental health counseling.
• Comfort and proficiency conducting therapy sessions via video conferencing tools.

• A minimum of 2 years of relevant experience in direct mental health service provision, ideally with individuals facing terminal illness.
• Experience working in remote or telehealth settings is highly advantageous.
• Demonstrated understanding of the unique emotional and psychological needs of hospice patients.
