• Conduct thorough clinical assessments of patients to evaluate their mental, emotional, and social needs, providing a comprehensive understanding of their circumstances.
• Create and implement individualized treatment plans using evidence-based solution-focused interventions that address diverse patient needs.
• Administer therapeutic interventions remotely, ensuring each patient receives personalized care that respects their unique journey and preferences.
• Maintain accurate and timely clinical documentation, adhering to organizational policies and regulatory standards to ensure the integrity of patient records.
• Collaborate closely with physicians, nurses, and support staff as part of a multidisciplinary team to coordinate holistic care for patients and their families.
• Engage in case consultations and interdisciplinary team meetings to share insights and strategies that enhance patient outcomes.
• Provide education and resources to patients and their families, fostering an understanding of mental health and coping strategies during challenging times.
• Remain current on developments in social work and hospice care through ongoing professional development and continuous education.

As an LCSW within our hospice center, you will work primarily with patients facing life-limiting illnesses and their families. This vulnerable population requires specialized care that addresses not only the medical aspect but also emotional, psychological, and spiritual support during significant transitions. Your role will be critical in facilitating open discussions about end-of-life options and supporting families through bereavement.

• A Master’s degree in Social Work from an accredited institution is required.
• A valid LCSW license in the state of Virginia.
• Minimum of 2 years of clinical experience, particularly in hospice care, palliative care, or similar settings.
• Strong knowledge of solution-based therapeutic interventions and an ability to apply them in practice.
• Excellent communication and interpersonal skills, capable of establishing trust and rapport with patients and families.
• A collaborative mindset that fosters teamwork in a remote work environment.

While a minimum of 2 years experience is required, we prefer candidates who have at least 3 years of clinical experience, especially those who have specialized in psychotherapy tailored to end-of-life issues. Familiarity with electronic health records (EHR) and telehealth technologies will be beneficial for managing patient information efficiently.

The salary for this position ranges from USD 300,000 to 500,000 annually, commensurate with experience.
